I am in the 50-75 %. Lots of good things can happen above ground too, but it seems that the bad guys always want to hide underground. The key is to make each dungeon crawl a little different.

Don't put the BBEG in the city. Put it far outside the city in a long lost temple. Once the PC's find it , they won't have time to travel back. They will have to deal with it right then.

wishes were not too common in my campaign. Each wish was granted by a higher power, either good or evil. Sometimes the evil ones would require a favor or some kind of repayment. I remember a character who got 1 wish granted around 6th level and never used it. Retired as 19th level Ranger.

I have been running a player with the FB for about a year. 10 total levels now, not sure how many FB levels. I think its 4 though. A couple of times he has hit a few characters but never that bad. If the party is still doing well, the monk will go looking for the next battle and lead the FB...

I've done it a couple of times and worked out great. Each player had 2 characters with a back story as to why the two adventured together. The first time was 1ed Ranger and Cleric/Magic user. Next time was 2nd edition Wizard and Bard. I tried 3 characters one time and that did not work as well...
