My impression is that this is fairly common. Not >50% common, but common nontheless. I like your advancement rate, personally. Keeps things fresh. So long as you don't run out of levels before you run out of campaign.

Their business plans seems to be reasonable to me. I can't think of anything in particular I would change. I definitely wouldn't aim for a whole new direction.

Torture and violence do not work that well. Victims subject to these methods will say anything to get it to stop. The difference between getting anything and getting the truth is key. If a player fails intimidation and resorts to violence, he gets lies.

This seems like one of those questions you'll never find an answer too. However, I'd reason that it's more profitable than even the books.
