    Definitely a CoC book. I have it, but not readily accessible. I believe it was the 3rd ed hardback CoC rulebook, which included the Cthulhu Companion and a third book. The book included several other prisons. Give me a day and I'll dig it out.

    I go for the Aztec/Nahuatl pronunciation (with x=sh) as well. It looks Aztec anyway, even if nothing to do with Aztec mythology, and it's quite easy to say. I-shi-cha-chi-tl. Who invented it anyway? Gygax?
