For me (and my players), I think that the method I use should be determined by the type of monster. Gnolls, while they can work together, just don't seem organized enough to me to pull off such coordinated tactics. Thus, option 1) would apply. OTOH, Hobgoblins? Definitely attack like option...

Also, Saitou, if those ranges exist, what better way to sell the MM to those who make all of their own monsters than putting that data in the MM itself? Get those rules, and by inspection all of the various 'categorized exceptions' in there as well.
