Only one small change to combat and skills. No changes to feats, but a few have been added. Lots of additions to basic PC races (over 20). Magic rules have been adjusted, but more concerning individual spells rather than the spell list, rules per casting, etc.

Aprox every three months at lower levels (under 12th). I love the pacing, as it gives me lots of time for them to work on plots. Adventures are essentially missions, and I give out bonuses for completing these missions or personal goals. But they also get adventure and RP rewards.

I'm a 4th level Nerd. Enough to be noticed, not enough to be bothersome.

Which is exactly what I have done in my campaign world. Works great, especially for players who recognize it from previous planescape gaming.
