    Or better yet Jaracove, use the actual encounter building guidelines in the DMG. CR's can be deceptive and are not they way to build an encounter. Use the XP budgeting guidelines (with the multipliers) that are suggested in the rules.

    Yes, with a grappler feat. Also, some monsters have special grapple attacks that impose the restrained condition.

    Now that I am thinking about it, I've rarely seen concentration broken by failed Con saves in our games. More frequently it is broken when another concentration spell is cast.

    +3 weapon is legendary. In fact, you can destroy artifacts with it. I wouldn't worry to much about parties having +3 weapons and ammo unless the DM is an idiot. (or the party is epic levels).

    Ilbran, so if a PC in your game rolls a natural 1 on an attack roll they suffer an opportunity attack? What a horrible rule. High level fighters with 4+ attacks per round (double with action surge) will be getting negative consequences less than every 30 seconds in combat, on average.

    Most aren't. Most don't have a jacked up wisdom either. They usually take expertise in one skill and thieves tools. Again, just feels like a "default" rogue should be better at finding traps without having to give up other options.
