Question: in his stats it says his horrific appearance causes (DC 369, insanity). Does this mean you go permanently insane if you fail the save? And how are the gods not all insane from when they fought him.

I'd say Elder Ones, since they are all different parts of the planet and sky. I was thinking instead of outsiders, they could just be epic great wyrms with the elder one template. Maybe have Siberys as a platinum dragon, Eberron as a titanium dragon, and Khyber as a polychromatic dragon.
