Basically you want to detect a disease before its dealt any dmg, ie, during the incubation period ? House Rule Territory -- Maybe a heal check would be suffcient? Couldnt find anything in the SRD. Or add a 0th lvl Detect Disease, since there's already a detect poison.

I've got an NPC barbarian that I've put with the party that, storywise has "trollblood" in his ancestry and thus has regeneration 1. How powerful of an ability is this, as far as level adjustment? +1 ? +2 ?

I would vote No, and No. The entry on breathe weapons says if you have more than one breathe weapon then you still have to wait 1d4 rounds, if you need a little rules-backing.

Or you simply hit the head with the ray instead of the body. Less work for the ray to get to the brain == more dmg. Or you hit the ray right in the spine and it shoots up to the brain quicker. Plenty of ways to justify :)

Huge has a speed of 100ft(perfect)...If he's got the height I don't see why he couldnt pick up an entire party. Although you should still be able to attack the elemental while it's got you picked up. so something like(where X indicates a medium sized creature.... 50-X X X X X X 45- X X X X X...

Technically, I don't think it would affect the HD limit since its not mentioned in the spell, but alternatively I'm assuming you could suppress your fear aura and simply use the abilities of the spell. Except that would suck because it just makes enemies shaken :( Not all that great for a 6th...
