    It's not a problem unless you assume every hit opens a wound. Some might hit armor or with the flat hard enough to cause a nasty bruise. Or helmet and cause a minor concussion. Even a simple (partial) bone fracture somewhere where it isn't debilitating might heal in the max two weeks it takes...

    This isn't a problem if you interpret HP as physical damage, but 1 HP as different for different targets. 10 damage to an uninjured 10th level fighter is a minor flesh wound, while 10 damage to a 1st level fighter is a serious wound that will take a week to heal.

    That's not the balance this thread is about. The DM can always adjust to a strong party by upping threat level. If one character dies every combat and another never takes a hit, then there may be cause for concern.

    In which edition was this the case? Depending on edition at least Sembia and the Border Kingdoms were intentionally left there as white spots. There were also several "grey" areas of which there were few details. The 3e FRCS had a couple of mysteries for every region specifically as plot hooks.

    Depending on campaign aasimar and tieflings may be. Also drow, genasi and some "monster" races have been seen (at least centaur comes to mind). The others don't exist in 3.5 Realms.

    That just means you are/that school of though is Chaotic. :) But seriously, the system doesn't have to be rigorous and chaos/law axis doesn't have to be completely independent of good/evil for it to work in D&D.

    Like in our folklore, the things they "know" about any but the most common threats may vary a lot. Using the 3e Knowledge rules, untrained characters have no chance to identify monsters they haven't dealt with before. But this is a campaign issue.
