    FWIW, a teleporter that only sends part of something is just as deadly as a disintegrator. I can imagine a PC ending up on the other side, cut into 100 narrow fillets as his momentum carried the already dead body through the maw. Bad choice, either way.

    It's got to be Castle Greyhawk. Not "classic" by any stretch, and the worst bit of dreck ever to receive a stat block. After that, any "classic era" module for character levels 20+. Even Isle of the Ape was terrible in this regard.
