In real world hermetic occult lore, gnomes are really earth elementals. They're not physical entities at all but spiritual. Same with sylphs (air) fire (salamander) and water (undines) but those last three aren't even options as PC races unless you're using Savage Species.

I don't know why DnD thinks it needed a halfling/dwarf hybrid: the gnome. I think it's a pretty useless race, and 3.5 made it even LESS appealing with bard preferred class. Now we have singing halfling hybrids wearing pretty silks.

I always assumed they ceased to exist as they melded with the plane of their final resting place. Probably not even resurrection would bring them back. Maybe not even divine intervention. I'm not sure how Manual of the Planes addresses that question.
