    A couple of other common Dodge uses at our table include Patient Defense (aka Monk tanking) and combining with Spirit Guardians (cast and then subsequently dodge while moving forward to just let the spell melt everything).

    There's no right way to do these things, but one reason we've never expanded past one-shot or wounded for our minion rules use ease of tracking. We just toss a little red plastic bead on the map next to the wounded monsters, effectively removing the bookkeeping.

    We use a pseudo-minion rule, which is basically if you don't one shot the monster it dies the next time it takes damage. No hard and fast rule as to what kind of monsters are minions, but generally orcs and other "support" type monsters once the characters are of sufficiently high level.

    If the players don't care about money, it's because they have too much of it. They either have too much of it because the DM has been too generous or because there's nothing they need to spend it on (or somewhere in between).
