It could be something to do with the 5e conversion. I tried to make the 5e version the same intent without breaking 5e action economy. Maybe the lich inquisitors were out of channel divinity uses? Using counterspell or dispel magic against the fly would have been a bummer.

Yes, in adventure 5. Check the timeline in the campaign guide. I would recommend reading the rest of the campaign as you are able, even if it is just the introduction at the beginning of each adventure. EN Publishing's campaigns reward you richly for having that extra bit of preparation.
