The PHB is for player knowledge. The DMG is for DM's knowledge. Knowledge of a class belongs to the player. However, knowledge of magic items and how they work belong to the DM. You can play however you wish though.

I would say both. Not all sources of spells are scrolls. Some are from other spell books.

In regards to keeping in the spirit of 5e, I hope they include concentration. It goes a long way in reducing the overpowering effect of spellcasters.

I'm doing a mashup. Combining N1 with U1 and a couple other one-off's. So, by the time the players head to the naga's lair, all seven of them will be 3rd level. It will be tough, but they should be able to handle it. There won't be a Ramne there to use as a crutch either.
