I think Initiative based on Int is a great idea. The game would need a little tweaking to make this work with certain feats and subclass abilities but would be a good way to balance out the big advantage Dex has.

Don't worry about it. Int is associated with lots of skills. Emphasize that more knowledgeable characters get more info on pertinent skills than those with bricks in their head.

If allowed, mix up boss and mini-boss abilities. Toss in some PC class abilities - a troll paladin in plate/shield wielding a greatsword one-handed and adding in smites is a fearsome thing indeed.

I generally remind players that Wish is a reality-bending spell with pretty narrow effect. If it wasn't, wouldn't every lich out there Wish for world domination every day? The narrower the focus, the more coherent the Wish and the wider you spread it the more it dissipates to nothingness.

I allow players to design their paladins and fill in how they serve their god or purpose. Religions have lots of dogmatic splits in them and the PC helps me world-build by defining their order.
