    Sounds like a problem of scenario design then. The DM is supposed to set a timer so people start to die if the disease goes untreated. It's up to the party to come up with a solution for the scenario.

    Wr have been doing that for ages, without the need for any special systems. This is just the natural development of any campaign with a fixed home base.

    I strongly disagree. DMs are not professional game designers. Give me some official variants, please. It doesn't have to be exhaustive and cover every little situation, but at least something to tweak pacing and difficulty.

    That's my main concern too. Variant rules are the most useful part of the current DMG for me. It rather looks like this new DMG is players focused, since they are including a new magic items crafting system and the new Bastion system.
