RE: Hemlock I have ruled that if a player is killed or reduced to 0 by the Balor death burst, that the character's soul is taken by the Balor and the character cannot be raised or healed unless a deal is made. This at least gives the Balor something of an incentive to fight it out to the end.
In my game, if the summons is successful the creature waits to be given an order or task. If the summons fails and the creature still appears it is free to do whatever it wishes to do.
In my area there are very few fighters, monks or barbarians. If someone wants to do one of those there are HEMA groups, Society for Creative Anachronism, and self defense clubs. People want to play something they cannot do in real life.
I have always had the dead buried in the church yard of the church they worshiped at. With the wealthier being buried either inside the crypts and vaults and the poorer being further out on church property.
For your players the "little people" may be more important. The halfling mage in our game tends to ignore the big players, her best friend in the city is Fritzroy the Falconer who is in charge of pest control. The two of them have a great time atop the city spires eating rabbit and pigeon pie.
I tell every one of my players that anything that might happen in the real world, that is either seen on television or read about in the paper can happen in the game. Whether or not it happens depends on if they go out looking for it or place themselves in a particular situation.
Use the terrain. Small piping where players have to crawl rather than walk. Tight areas to squeeze through, "Sorry have to take your armor off to get through". A flash flood due to a heavy storm.
There are also the invisible unseen beasts called diseases.
