I think I’m going to use a stand and fight mechanic - once a PC has dropped to 0HP, they start making death saves at DC15. They can keep fighting, however, but can only use an action or a bonus action, no reactions, no concentration spells, and move half their speed.

Is ½ enemy HP and 2x the enemy damage too extreme?

Do you have anything like Concentration on those save spells, where it can be broken and free the character from its effect?

It’s more about pacing than world building. Everything’s gotta go fast and be quippy or you’ll lose audience attention because attention span is so short and expensive now.

Of course, but for new DMs who join and don't have legacy knowledge and aren't terminally online? They'll be entirely unaware.

Unfortunately they don’t have a trend in 5e of releasing any purely DM-facing material like a book of variant rules would be, so I think getting them in the DMG is likely our last hope.
