XP For Defeating a Creature Twice

This is probably a more radical (and less helpful) suggestion than you want, but have you considered not awarding XP for combat? A few years ago, I started awarding flat amounts of XP per session in my games so as to keep PCs advancing at a speed I'm comfortable with, rather than doing so based...

    Template for a troll

Also, if you're someone who believes in using the mechanics provided by WotC and using the flavor that best suits your game, then the original fluff for the template doesn't really matter. Find a flavor description that fits the abilities and suits your taste, and run with it.

I've seen that happen with a paladin and his mount. It's beautiful :D!

    D&D 4E Verisimilitude IMPROVEMENTS in 4e

Depends on what the game system is trying to achieve. For me, a 20th level fighter is on the far side of mythical reality. Would a fall off a hundred foot cliff kill Cuchulainn or Bhima? Hell, no! And that's precisely what a 20th level fighter is to me.

    Stupidest things PCs/DMs have done

Semantics. But then again I've never been one of those DMs who figure that when someone uses a Wish they must focus on the minutiae of his wording and screw him by hook or crook. When someone's using a 9th lvl spell, and in this case one in a very valuable item, I'm damned if I'd screw him...

    Combat is faster - right?

Absolutely. I have no problem at all with an exciting combat that spans a couple of hours. It's when the exciting combat spanning a couple of hours is actually taking 3 rounds of game time that I have an issue with it.

Camping: It does a body good!

You've already answered the question in your first sentence. Your PC is apparently able to survive being hit by 3 fireballs, each of which is so hot it can "melt metals ... such as lead, gold, copper, silver, and bronze" (3.5 PHB). And he can take one of those and walk away from it. Clearly your...

Kio'oukath Spearbearer: Male goliath hoplite/arbalestier [please critique]

As noted by Kat', and it does seem to be true for all your characters, it's very much a glass cannon. Lots of ability to inflict damage, but very limited ability to absorb damage. A low-level spellcaster would murder the guy without any effort, and a really melee-oriented PC of 18th lvl or so...

    Template for a troll

That's what I would have suggested. I should also note that the Voidmind template is horribly under-CRed. It should be at least a CR +2. Another option from the same book is the Woodling template. CR +2.

    New Giants Minipack Name Sucks

Don't soil the glorious modules from yesteryear? That's a little too much worship at the fane of modules past, IMNSHO. It's just a name.

    Ampersand: Sneak Attack

:D You know, I think that's one of the reasons why my sense of disbelief seems to be much more robust than many other posters'. If I can deal with the idea that I live on a rock which flies around a giant flaming ball, both of which are hanging in space, I can deal with a whole lot of the stuff...

    Ampersand: Sneak Attack

I think it's more likely just a reference to the character in Dickens' Oliver Twist. His name is admittedly working off the same pun, but since he essentially added that phrase to the lexicon even for those who don't know cockney rhyming slang, and it means someone who's good at, well, dodging...

What are your favorite RBDM tricks?

Another fun variation on that is medusas with greater invisibility. It's really funny when the mage casts See Invisibility, says "Oh, shi...," and turns to stone. For a certain value of funny ;)

    Tweaking the difficulty level.

Your methods all sound fine. As suggested by others, adding a few creatures when possible is a good idea too. I recommend starting out with the only tweaks being max hp and changing feat selections to make them more effective (just using Ability Focus can make a creature significantly tougher a...

    Detect Evil on an Embryo

You know that Detect Evil can penetrate barriers, right? You need a foot of stone or an inch of common metal (other than lead), or 3 feet of wood or dirt to block it. I'd say it depends purely on what alignment means in the given campaign and how early it manifests. Personally, I like detect...

    Hemispherical Wall of Ice

I'll disagree with Ogrork the Mighty. I treat it exactly the way you do, Volaran. As written, I think you can catch a creature without a save as long as you center the hemisphere appropriately. And no, I don't think that's too powerful for a 4th level spell.

    How do you do NPCs?

:o Glad to help. The more I DM, the more I find that having less preconceptions about what'll work and more playing off player/PC reactions is the way to go. At least for me. Personally, I think it's just fine if they don't take him seriously. He can still be a serious annoyance even without...
