There is no universal exception, but many of the more recent abilities to grant DR X/- explicitly say they stack with any other DR X/- that the character may already have. The DR granted by the Iron Mind prestige class (Races of Stone) stacks, for example.

How do you figure? The goliath greathammer's damage is only one step better than the nearest equivelent weapon, the scythe. Acually, it's less than a full step; 1d8 upgrading to 2d6 gives an average increase of 2.5 points, whereas the goliath greathammer's (1d12) average damage is only 1.5...

I'm not seeing that. You can make a "melee attack" as part of a Standard or Full-round action and i see nothing here that implies you must make your melee attack to recover as a Standard action. In fact, it seems pretty carefully worded so as not to make that implication.

The monk, on the other hand, is going to wish he spent his cash on the bracers. ;) But yes, for arcane casters there is very little reason to ever get bracers of armour.

Complete Warrior has the heavy poleaxe; 2d6, 20/x3, S or P, reach, set vs. charge. Pretty much exactly what you're looking for; the only thing missing is the tripping ability.
