Huh. Interesting. I was only counting spells like detect chaos/evil/good/law/funky once, instead of four times each, but I'm still at 600. Either I'm missing 5 or they're including 5 that aren't in the PHB. I'll have to figure out which it is. :)

Incorrect! Read the "Special" section. You get the benefit every time you meet the prerequisites. There are three sets of prereqs. The sample character meets them all. This is NOT an arbitrary thing; this is the very core of the question at hand. This thread would not be as long as it is...

It's been a while since I calculated it out -- and my harddrive is a disastrous mess -- but that sounds about right. The average save isn't arbitrary, but it is not an obvious progression. Of all the answers I've heard, "It sounded good at the time" sounds the most logical to me. :)

For place names, I use historical atlases. I am usually stealing a culture, so why not steal the place names? Most of the names have changed over time and, if you pick smaller places, you can easily get names that no one other than a scholar of the culture has ever heard of. :)
