I have never used a small shield or seen anyone use one in the year I have been playing D&D, with 4 different groups. Have you or anyone you have played with used one? What was the reasoning behind it?

Maybe untrained characters should negs to their checks and not be allowed to take 20 (or 10) then.

One of the campaigns I'm playing in has been running for 2-3 moths, and so far we've lost 11 PCs. You get rather used to deaths in the party playing in games like this.

You can take 20 to tie a rope, because failing the check brings no immediate consequences. Failing the check only carries a consequence if you actually use it.

There unfortunately aren't any PrCs specifically for characters using double weapons (unless you happen to be an orc or half-orc). The Weapon Master from S&F is about the closest thing you'll see, and that is quite feat intensive. Regarding feats, improved critical is nice. Aside from that...

If you know for a fact that your opponent will be good or evil, I would recommend going paladin or blackguard. Get as many smites as possible, and take the epic smiting feats. If you do it right you'll be able to deal disgusting amounts of damage.
