    As the DM, it's completely within your rights to rule it any way that makes the most sense to you. But from a gameplay perspective, having it generate an AoO doesn't really seem to put a tick in the "fun" column imho. Your party has just had a character drop--not exactly fun, but it does...

    I wouldn't house-rule a heal check to be subject to AoO. The PCs have already been penalized with the (however temporary) loss of a character, not to mention the loss of an action by the healing character. It's enough I think.

    Completely agreed, the MM3 is fantastic, and the elementals are great design. The ability for a (greater?) fire elemental to "flicker" (shift) is just fantastic imagery.
