Eladrin are chained to the wall more than 6 squares from the bars. He can teleport out of the chains, but a guard will catch him before he can finish his short rest and teleport out of the cell. If he has more teleport powers, then I guess he gets out.

It works for some campaigns, but it's not my preferred way to play.

Generally, my group will pause the narrative at the start of combat, play through the combat round-by-round, then when the combat ends, go back and redescribe the combat as it fits into the narrative, then continue on with the story.
