    If you can find it, there is a spiffy, though short d20 PA game- Redline from Fantasy Flight (part of the Horizon series). More Road Warrior with optional mutant rules than anything else. I don't own it, but have heard good things about OcTane from Memento Mori Theatricks. It is both print and pdf.

    Apathy in a majority of the population is fun to play with (and not totally unrealistic), but my favorites are disease and alien invasions ala The Screwfly Solution (ie there isn't a lot of gaping holes in the ground).

    Benefical ones are rare, but not unknown. Extra fingers*, better eyesight, either larger jaws or less teeth, color changes (dark skin does help in regions with lots of sunlight), slight increases to longevity among others. If you want extreme, then implants and symbionts are the way to go...

    Phantom fungus. It is easy enough to increase its intelligence and give it class levels or other templates to increase the power of individuals. The natural invisiblity is a good way to freak out players. Or go with phasms- the moon men have engineered themselves to work in any situation...
