    I was reading my sources, and I can assure you the Ethereal Plane didn't existed in 4e. At least, conceptually. The authors purposefully left the Ethereal Plane out of the World Axis because they believed it was an unnecessary addition to the cosmology. There is a Dragon article about it...

    As with anything Nentir Vale, there is no single answer. It can be the Living Gate, but other sources say it can the very first abolethic colony in the universe (according to the legend, the Primordials built the Word on top of that) or whatever something it is that created the very first beholders.

    I only wonder if Io is mentioned at all in the book. But I guess that the lore from the SCAG is still true for them. It would explain why Bahamut and Tiamat are that important now.

    Well, these are aspects of Tiamat and Bahamut, not proper avatars. In 4e, the aspects were weaker than the avatars, so I guess I was expecting something like this.
