    No. One of the major changes in 3.5 was to delete that hierarchy that existed from 1st-3rd Edition. The rationale was sort-of along the lines of "when fighting lycanthropes we want you to really need silver". Others can probably give more insight to the theory behind it.

    Yes and Yes. Note that 8th-caster level wands of fireball and lightning bolt are actually listed as standard treasure in the "Table: Wands" of the DMG. I presume you know they cost 18,000 gp, while a 5th-caster level one costs 11,250 gp.

    I do. My assumption with invisibility is that it allows you to get right in someone's face and attack with impunity (hence the attack bonus). Hiding's not the same -- I would really need a specific rule that explicitly says "hiding = invisibility", and there's never been one, not in RAW...
