    Nope. I tailor things to fit the players and characters. I like D&D as heroic fantasy adventure, not survival horror Ocean's Eleven.     Thinking about it more, ranged characters are still going to have an advantage on initiative because they usually don't have as much of a reason to move vs a melee character.

    I'd probably just make it a racial feat that gives them a tail and lets them use it as a natural weapon. Probably a d6. I like to think dragonborn have some sort of myth about how their ancestors lost their tails.
