Think of it as an area: You start out with a 1 inch². If you widen the square by 1 inch, you get 2 inches² (1X2). If you lengthen the square by 1 inch, you also get 2 inches² (2X1). If you widen AND lenghten the square, you now have 4 inches² :) Slim

For headbutts, treating it like spiked armor is fine. If the player wants to charge foes to skewer them in the abdomen, you could give it double damage on the charge, but the charge will provoke AoO's (since the dwarf is looking at the floor). Slim
