I've run games at 1st, 30th, and everywhere in between. Other than requiring a bit more forethought for encounters and challenges, I don't think that the preparation time is much more for higher level games. Of course, I don't even pretend to try to look through every book to customize...

I've had a few bad players, but no one to this level! I definitely wouldn't have any bad feelings about booting him out, and I'd certainly approach the other GM about doing so as well.

I agree that it helps keep track of what the items actually are, as Teemu stated, but for in game flavor, I prefer using Potion of Healing or other less-crunchy descriptions. Likewise, I never hand out generic +2 keen swords - items like that are always singularly made items with other features...

It's always been my favorite holiday, and I always try to do more and more to celebrate it each year. Between the decorations, the trick-or-treaters, the scary movies, and the entire vibe of the holiday - it's just a perfect mix of joy with a touch of the macabre thrown in. Delicious.
