It's my favorite WOTC book in quite a while as well. Lots of goodness within to think about and examine how to and whether to fit into my campaign.
And I agree with what's been said before; yes, I'm sure a lot of it has appeared in other d20 products...
As a DM, I prefer one or two big items, as it's much easier to keep up with and plan that way :)
As a player, I like to have magic trinkets dripping off me like price tags hanging off Minnie Pearl.
No problem as a player with commisioning items (ala Dragonslayer, Princess Bride) but I don't like magic shops. I would enjoy (and have used as a DM) the idea of a magic black market like the current antiquities black market, as there are a lot of tomb raid... er... I mean adventurer parties out...
You could use an "other" or "mixed" choice :) ; I buy my WOTC books and a few rare other books that the local B & N carries because it's 2 minutes from my front door and I have my 10% discount; almost all the rest of my buying comes from my FLGS across town (nice people) with a very few...
We play in the spare bedroom in my house, "my office" per my wife :) . It has a folding table, folding (cushioned) metal chairs, my computer for record keeping/illustration/music/sound fx, and lots of shelves for books, minis, terrain, etc.
I think it must be because I went from playing 1e to a long hiatus to 3e that this seems quite natural to me. Characters have always had backgrounds, stories, personalities, etc. in our games but have also always been like the players KOTD looking for that Hackmaster +12 or Crossbow of Doom...
Just look upon it as a lesson why not to quit taking your medication without asking your doctor first.

Nope, that's one quirk of the hobby I never understood or was infected by. I have a bag with about a dozen assorted sets inside, and dump them out at the game for anyone to use, after picking out a set to place behind my DM screen (and they aren't my "eeeeviiiiiil" set, either :) ).
I'd agree it's a little long and a little too formal, but otherwise I think it's a very good thing to get all this out in the open up front; it certainly lets anyone know what to expect. As far as points people have raised:

And remember that monsters guard treasure; they never use it; always preferring their tattered leather armor and rusty sword over the elfin mail and +5 sword in the chest they are guarding. Or better yet, hidden in the hole underneath their bedroll.

Thought of annoyance finally; players who don't read the (very short, paragraph long) synopsis emailed out before the game if it's been a while since the last session, and then can't remember what happened last.
I've been very surprised not to see a single mention of facing rules in this thread, particularly from the "square bases are ruining D&D" faction. Is anyone using them, do they feel the rules just don't measure up, or have people simply decided square bases and no facing wasn't such a crucial...
My greatest annoyance is having a stable, well-balanced group of players that is fun to be around and never gives me any grist for posts like these :( :(
DMs whose personal pet peeve leads them to screw over the player group; e.g. they hate high fantasy/high magic so at 5th level you're lucky to have a +1 sword and a couple hundred gold, but then fail to take that into account when they throw high CR monsters at you.

Much the same as what's been put forth already: anything that takes away something that they feel they've worked hard to earn. This would include level draining, stat loss, and critters that steal/destroy/sunder treasured weapons and items.
I've never used cut scenes to give away future events or detail what is currently going on elsewhere with the BBEG; I often use them in my write ups for my players when I convert the session into a story. As they approach the door to the evil fortress and make their stealth roll, I will cut to...
