I disagree. I believe it is only arcane & divine making 4 the maximum in the OP's example. The druid spells would fall under divine. Essentiually, a druid / cleric combo would never qualify twice because BOTH are divine casters.

Glancing at the SRD, I'd say , Yes.

Although I'm not into FR any more, IMO these are three of the best books TST / WotC ever published. Another good pair. Temple of Elemental Evil / Scourge of the Slave Lords / Queen of Spiders is a GREAT and classic series. As an old-school gamer, I recommend these to anyone the has never ran...

Which issue?

Toss up between Nerull & Wee Jas. Wee Jas does the the magic thing for a boost, though. Wee Jas, followed by Nerull. The Harvester of Souls taking third.
