    My understanding is that you can not use both the spear and the mace in a single attack. You can hold both, but you can't wield both.

    NPCs don't have to follow the rules exactly... if these doors are specially designed to use this style attack, they can have a mechanism that allows it to be a free action, or whatever works... Make it interesting and the PCs won't think they're getting screwed over...

    I'd say no. The extra radiant damage is from the zone created by ToL. OS does not gain the Radiant keyword.

    Depends on who does the moving. If they close with the PC, then the PC can't shift and run unless they can shift more than one square. That's velcro-like.

    Sure... why not... it's hardly overpowered. If you want the rod to keep you off the ground you have to hang on to it... so not really flying per se... ... so a level 10 Warlock could work there way up a cliff or over a gap... but watch out if their artillery sees you. There are easier ways.

    That's only true of alchemical poisons though. There are powers and other items that do poison damage. Besides, the best part about 4e is that the NPCs don't have to follow the same rules as the PCs. I would just say that the NPCs are just better at it than the PCs are, and if they want to...

    It's an epic level power. I don't think it needs to be limited more than the existing DM ability to deny free actions.

    True, but that doesn't give the DM or the player any guidance as to whether to allow the NPC to finish their triggering attack before allowing the Free action.

    Thanks. I really wish they would have specified whether the free action granted was an interrupt or a reaction. Other than that, it seems straight forward. If an opponent starts next to you, attacks you, and sticks around, you get three free attacks.
