Imagine being inside a big plastic ball in the dark, put a few spots of "vital organs" on the outside of imagined ball and while people roll the ball around you try stabbing those vital organs.

Polymorph gives you the physical attack type of the creature not the "attack routine".
The fighter would use his normal BAB and can take the appropriate penalties to use a secondary attack as an off-hand weapon.
Wildshape gives you the attack routine regardless of BAB.
5 off of d3+10d6 isn't going to help much.
Sneak Attack bonus damage is applied for each attack that qualifies just like a Flaming weapon does d6 of fire damage even if the physical blow doesn't deal damage.
So when a description reads that it may seems unbalancing or contrary to other rules it's "flavor text" but when Barbarian DR says another it's played exactly as worded.
Why? Because
A. it's not unbalanceing?
B. that's how it reads and it's obviously meant that way and not flavor text?
C...
