    That's too brutal. You could have a "light load" and be unable to swim. 5lb isn't much in the water. With that penalty, lifeguards dragging victims would drown like half the time. And, yes, you can swim carrying a cannonball.

    I agree with Empirate Here is what I suggest: let him wish for "regeneration" and then you, as DM, can set the parameters based on what you think is fair given the power of the spell. Be it regen 0.5, regen 5 or regen 50.
