I worked it that a small or unarmoured character could just squeeze through the gaps in the gears. That meant that the entire party couldn't bypass the puzzle, but the smallest character could set the gears appropriately and then climb around the outside of the gears until they rejoined the party.

Also, at first level there it's much more likely that improvisation will happen as characters don't have a huge menu of options. That can lead to some very early character development, shaping the whole campaign.

I love starting my campaigns at first level. There is a much better level of fear and frustration which pays off as the characters get more powerful.

They all stayed dead. In every case the other characters have escaped but got no chance to get the body, or did not have the resources or access to get such powerful magic.

I just don't understand the assumption that characters don't die in 5e. Every one of my campaigns has had at least one death and most had several. I play rules as written and it can get very hairy from time to time.
