I'd rule that page 200 is correct. If they can't be bothered to put all the information about something in one place /and/ make it clear, I can't be bothered to try and figure out what they ment. The OP is also forgetting that for quite a few campaigns, dropping dead at level 10+ isn't necessarily the end of the character, what with Raise Dead and all. (And when did our world hit 1 billion? Sometime in the 1800s?)
