    My experience has been the opposite of that in the OP. Thinking back on past campaigns, those that have had the most staying power have been ones without a strong aspect of good versus evil or much in the way of an overarching plot.

    I based it (more or less) off of the books which grant an inherent bonus to an ability score. The main difference being that I use a cubic wealth progression in the games that I run, so cubing stuff is sort of my primary method of determining how much stuff should cost.

    In the games that I've run and played in, I don't find character death to be a problem. Making players sit out of the game for a long period of time can be a problem (as mentioned by Morrus, above), but that isn't necessarily a result of character death.
