    Create characters together. It's the best way I know of--you don't just end up with guys who met in a bar then struck out to kill a dragon together, or people with "Oh yeah. I guess he can be my cousin or something." tacked onto their personality, but a group with intertwining goals...

    Damn. He (the dm) had SOOOOOO many chances to throw some cool plot twists, make interesting adventures, have you encounter neat NPCs, and in general run a really sweet game based on players taking the initiative. And he threw it all away. Find someone to GM who won't make those mistakes...

    Are you a "loose" DM?

    Loose with the rules? Extremely. With the loot? I wouldn't know. I pretty much ignore the GP values of everything, since most of them are arbitrary anyway, and just compare raw stats after mods to find appropriate challenges.
