I just assumed that the "usual" method was just to use the book price? I always do this when I DM just to keep things simple (I don't want to dig out a calculator when someone wants to buy something).

Because the shackels have a magical enchantment on them that cause any magic cast by the person wearing them to get hurt by their own spell. Also prevents any benificail magical effects from taking affect as well (teleports, spider climb, etc else would be useful for getting out of a prison).

You could also relate it to the glyphs (proper term is escaping me at the moment) on the warforged as well if you wanted to.

I always envisioned the ooze to basically get thinner when it splits as opposed to significant loss of diameter). Say it starts out about 5 feet high and ends up about 2 1/2 feet high after a split. All a matter of how you look at it :)

Belt of Vigor (PHB) also increases you healing surge value by 1, 2 or 3 points depending on the level of the belt. According to one of my player's wish lists its on page 253.

That specific issue was discussed shortly after release. I think someone got a replay from Customer service that indicated that Tieflings do age the same as humans.

If you were a melee build then Prime shot would obviously come into play a lot more than the ranged one.

The character gets 3 magic items (level+1, level and level-1) and an amount of gold equal to the vale of a magic item of level-1. I don't recall if this was in the PHB or the DMG.
