    I'm pretty sure that these all came out of one of the Books of Eldritch Might, by Monte Cook. I'm not sure which, as I used the Complete BoEM, which is all of them combined and updated to 3.5. I especially like Teleport Redirect. I do like the 4E idea of just limiting teleport.
