    I have a player that used phatasmal force to create a locked metal box surrounding a foe's head. They get saves but otherwise they are blind and in fear of suffocating. I allowed it on the fly, but am not sure about if this becomes a regular tactic.

    Any of the big monsters that "employ" lesser monsters seem like a good choice. Devils, dragons, and mind flayers are good examples of this. The current MM does a good job of suggesting underlings.

    That's a fair observation. But cons are a celebration of the hobby; grand spectacle is part of the equation. Not that WotC are doing it wrong, but that they recognize that when it comes to big brands and generating spectacle its either all-in or nothing.

    Other than the starting city (Neverwinter), most of LMoP takes place in and around the backwater town of Phandalin. It doesn't appear on the FR world map for 3e or 4e, and the town is pretty light on lore. IMHO, swap out Neverwinter and you should be good to go without making changes.
