    This is represented by Active (Search) or Passive Perception Checks in combat. Outside combat, the DM still uses stealth checks or can make the call to determine if circumstances allow for hiding based on narrative. That's how I would interpret this using Line of Sight rules in page 45 of the DMG

    Exactly. In a case like this, they can hide and hold the roll until enemies approach their location, I would then have the PC do the stealth check to see if they're successfully hidden or not. I really do not understand the issue; it's not rocket science

    Still, you should use the custom monster rules as the MM indicates on its first page. So, taking the previously mentioned Adrenaline Rush, you can add it to the monster and remove the THP component; after all, thats what the old Aggressive trait was.
