    The quick answer is, you don't. There are certain kinds of adventure, such as where the PC's need to search a large area for someone or something, that can get them out into the wilderness, but really, is there anything in the wilderness that can really threaten them?

    I find that my players are rarely interested in such trivia. I can improvise answers to such questions at the table if they come up.

    I wouldn't create a new mechanic. I'd give them various percentages that the critical/sneak doesn't work, like fortified armor.
