Borshak's Lair from Dungeoneer #3 - OD&D (1974), the description includes spell lists from Greyhawk referenced by no. Now looking to convert this to 3.5.

I went with 5-7 years which is actual time DMing, but if I just went on elapsed time since I first DMed it'd be over 20 years. I had a long break from gaming.

I'd personally vote for Lost City of Barakus - currently playing in this and do rate it highly, probably followed by The Grey Citadel - those two together should take PCs from 1st to about 8th. One small note though Grey Citadel is 3.0, but converting to 3.5 is trivial.

I'm very much in the 'lets see how this goes camp' - it could suck, it could be great, but on the whole I prefer it when its old PCs of other players that are in situations where the characters could interact with them, e.g. 'You enter the throne room of King Sigurd of Trafalgis....' or it's not...

I still used rolled encounters, but I will often tend to preroll the encounters themselves so that I've got stats and information available for things I need, but encounters can include non-combat things like weather conditions, merchant caravans and other travellers. As a player it depends on the DM, but I usually try and have something more than so-an-so is a <class name> from <town>, blah, blah as it can give the DM and player scope for creativity. As a DM I try and encourage it and use the backgrounds to help generate adventure hooks.

These days I still do it, but not to the same level of care and detail as in the OD&D days. In the main game I'm playing in the GM has drawn rough maps which the players can use rather than us having to try and draw a map from the description.

Okay - try and get into doing other stuff is the best thing - I have gadzillions of lead minis to paint so that I do instead or read, I'm in an area where the mild climate and common public transport make doing other stuff easy so I won't try and make too many suggestions dependent on those...

Max at first level for PCs - reduce the odds on dying a bit, subsequent levels roll (reroll allowed on first time a one comes up - if it comes up one again that's what you get)

Same here with both my groups (though with one it tends to be more to run other games as well as D&D like BRP CoC)

I'm glad to hear it, Captain Jack was a good character and as I think he wouldn't come back in the next Dr Who series due to scheduling problems.
