    Not for me; when I run published material I have to read it with a fine-toothed comb and keep it on hand to refer to during play. If it's my own stuff from my own head, then most of it's cached in RAM and my notes are just paged memory on disk, do be swapped in if referenced.

    That's fair. My thinking now is that a good campaign starts with a sandbox, and the initial adventure just gets things started. Maybe there's an overarching theme, but its interface with the players is almost incidental. Let's call these "framing stories" rather than campaign premises.

    Well, it's helpful that 1-20 correlate a little between 3.x/PF and 5E than 4E. I'd say, BAB/4 (round up) + 1.
