Can you take 20 in 4th edition?

If you don't want the fighter getting through the door, you either don't put a door there, use a lock stronger than the fighter's untrained score + 20, or apply some consequence to the roll such as a trap going off every time there's a failure. It's not an issue of taking 20 being autosuccess...

[High level monsters and powers] What can Graz'zt actually do?

Impossible. The only way to stop a DM from railroading your character is to not be at the table. No amount of rules for what Graz'zt (or any monster) can do out of combat will stop a DM intent on making sure his story gets told. If that's your problem, you need to find a new DM or switch to a...

    Cover and Secondary Attacks

By RAW it helps, if only because there's no exception given for the general rule. It can be rationalize it as the cover hampering the initial attack somehow. The arrow still hits, but not as deeply.

Can you take 20 in 4th edition?

Have the PCs ever noticed an ambush before it was sprung?

    Masterwork Armor, price and enhancement

Since nobody has provided a rule reference yet: So I guess it's technically possible to have non-masterwork armor of 16th level or higher, but it would be kinda foolish.

D&D 4E D&D 4E and psychology: Hit chance too low?

50 - 55% is good. It forces you to think. Any lower than that and the rare nature of bonuses in 4e makes it too hard to reliably hit your enemy. Any higher and you get back to a situation where standing toe-to-toe with an enemy and trading blows is a good idea and the standard tactic. In our...

D&D 4E D&D 4E and psychology: Hit chance too low?

If 4e hit chances are too low, you want to avoid Warhammer FRP like the plague. :)

Can the First Aid Skill "Use Second Wind" be used on unconscious characters?

You don't. If they're out of surges and you have no powers that let you heal them without spending a surge, they're screwed.

    Minions - tell the PCs?

I'm not talking about using monster knowledge rules to recognize minions, but rather to get an accurate description of the creatures in front of you. It sounded like if the player didn't make his check he'd not be able to see the rusty weapons and bad armor. As if perception requires knowledge.

    Minions - tell the PCs?

Why would you need to pass a knowledge check to know that they're scruffy, have rusty swords, and piecemeal armor?

    Minions - tell the PCs?

It depends on the circumstances. As a general rule I don't say it ahead of time, but since all of the monsters are described similarly, they figure it out pretty fast. If they have reason to know the opponent's capabilities before the fight, I'll tell them. For example, when talking with an NPC...

Magic Items in Pyramid of Shadows

Is there an excess of gold to compensate? Perhaps they're expecting the party to craft more items.

    Death Saves Out of Combat?

With no distractions and a DC of only 15, stabilization should be autosuccess for many characters anyway (and all characters after 12th level). Normally stabilization won't bring someone to 1hp, but that probably doesn't matter unless enough people were dropped that the rest of the party can't...

Black Dragon breathes from beneath water?

There's no mention of water breaking line of effect, so it's legality is completely up to the GM.

NPC Paladins and Lay on Hands

Has there ever been a perfect set of rules that covered every eventuality? Of course not. The authors do the best they can, and it's up to the groups to fill in the holes. The easy solution is to give the NPC the resources he needs. The harder solution is to complain about the authors being mere...

[High level monsters and powers] What can Graz'zt actually do?

There's an awful lot of information on him, his realm, and his goals if he's not meant to exist outside of combat. Just because he's lacking noncombat stats doesn't mean he only exists to fight and die. It means the designers wised up and realized that enemies don't need numbers except when they...

[High level monsters and powers] What can Graz'zt actually do?

If they give a rundown of what he can do outside of combat they both ruin his usefulness beyond what they think he should do and open the door for a wave of demands for the out of battle capabilities for every other major monster.

Thorn (Dungeon 160) Is really puny

What's a cr? (or in other words, there's no such thing as CR in 4e)

    Rogue at will powers ?

Usually AC vs. Reflex is going to be a difference of ~2 points. +2 to hit is a big boost in 4e, where bonuses to hit are rarer. If you fight an opponent with a larger difference between the two then you're even better off.
