    The only way to fix this problem IMO is to convince the content creators that there is profit (and by profit I mean $$) in depicting realistic consequences to violent action. I don't see appealing to anything else being enough to overcome inertia.

    Yeah, in a more realistic game Dex should affect all weapons, and Str should be used a minimum score for certain weapons. But Dex is already so powerful I'd rather mess with the ability score vs skill system than give it more power directly.

    Houserules are always an option (unless you're chained to organized play), and thus always matter. I've pretty much never not used them or had them on the table.

    We used psionics in 1e, but only rarely because you had to roll high enough. We did try every time. Personally I preferred the 1e psionicist class from Dragon #78 or the 2e psionicist from the Complete Psionics Handbook.

    The mindset required to play a narrative game in good faith (as player or GM) is hardly universal amongst the hobby, yes. I don't really have it, and neither I suspect do many of players.
