How about increasing the HD type they earn each level. As well as allowing them to choose one poor save and turn it into a good save.

The 1st printing of 1st edition ADnD include the Cthluth mythos. So wouldn't that count as the oldest quasi reference to a Far Realm. Since in that mythos the deties exsist ouside of time and space?

Also remember that Level Adjustment is not the same as Cohort Level Adjustment. I belive starting in MM3 they started adding that line to the monster stat information.
