    "Not all orcs" was built into the language of the 3.0e Monster Manual. Edit: Indeed, even the 2nd Ed Monstrous Manual noted the possibility of exceptions to alignment, albeit rarely.

    Reroll your entire pool at the start of every new adventure. What I actually do: you get max at 1st level, then have to take the average (round up) per level thereafter. (But also, add me to those who say there's no 'should' to this one - as long as DM and players are all happy, do what you want.)
